Anna Eleanor Roosevelt's maiden name was actually "Roosevelt". In fact, Franklin Delano Roosevelt (FDR) was actually a distant cousin of hers, as both were from the same prominent New York family. Eleanor Roosevelt did not have a different "median" name before marrying FDR as her maiden name was already Roosevelt.

Eleanor Roosevelt was born Anna Eleanor Roosevelt in New York City on October 11, 1884. She was the niece of President Theodore Roosevelt and the daughter of Elliott Roosevelt and Anna Hall Roosevelt. She married Franklin D. Roosevelt in 1905 and they had six children. She was an advocate for civil rights and women's rights and was a delegate to the United Nations. She was also the First Lady of the United States from 1933 to 1945.
